Teaching NeuroImages: Acute neurologic deficits due to surfer's myelopathy.

چکیده
A previously healthy 32-year-old man developed acute onset of lower extremity weakness (American Spinal Injury Association [ASIA]–motor L2-S1, grade 2/5), T8 sensory level, and loss of bowel and bladder control (ASIA Impairment Scale [AIS] C) while surfing. The patient was a novice surfer and recalled a prolonged period in prone hyperextension. MRI spine was consistent with surfer’s myelopathy, a rare nontraumatic cause of myelopathy found in novice surfers (figure). The etiology is postulated to be due to dynamic compression of the artery of Adamkiewicz. The patient recovered full function with conservative management over 3 days. Full recovery is expected in patients with AIS B or better, as opposed to patients with AIS A, who typically do not recover.

BACKGROUND AND PURPOSE Surfing is an uncommon cause of an acute nontraumatic myelopathy. This study describes the MR imaging characteristics and clinical correlates in 23 subjects with surfer's myelopathy. MATERIALS AND METHODS This was a retrospective review of 23 cases of surfer's myelopathy from 2003-2012. Spinal cord MR imaging characteristics and neurologic examinations with the use of t...

A 48-year-old man presented with ascending sensory deficits over 12 hours, followed by urinary retention. He had areflexia, mild lower extremity weakness, sensory ataxia, and a T2 sensory level. Smooth pursuit was impaired, but cranial nerves were otherwise normal. Diagnostic evaluation demonstrated CSF cytoalbuminologic dissociation and demyelinating polyneuropathy fulfilling the electrodiagno...

Adam Strzelczyk, MD Ulrich Sure, MD Felix Rosenow, MD A 34-year-old man with refractory epilepsy due to an arteriovenous malformation (AVM) of the right postcentral gyrus (figure, A) was evaluated for epilepsy surgery.1 After study of the size, location, and depth of the lesion, we decided to defer resection, radiosurgery, and embolization because of concern that intervention would result in ne...
متن کاملSurfer's myelopathy: case report and review.
Nontraumatic spinal cord injury from surfing is a new entity first described in 2004 and likely of ischemic etiology. We report the case of a 25-year-old man who presented to the emergency department with a 2-week history of lower extremity weakness after surfing in Indonesia. The patient reported developing low back pain, lower extremity weakness, sensory changes, and urinary retention shortly...
متن کاملSurfer's Myelopathy Mimicking Infectious Myelitis
Dear Editor, Surfer’s myelopathy (SM) is a rare, acute spinal injury that is presumed to involve the major blood vessel supplying the thoracolumbar segment of the spinal cord. The injury typically follows prolonged hyperextension of the back. This condition was coined SM because it is seen most often in beginner surfers. Here we describe a case of a patient with SM that required differentiation...
